In the last scene they're giving Dean a lethal injection. I was hoping to imply that he was literally out of his mind for a while--he wasn't really conscious of what he was doing and can't remember it afterward--and ended up killing one of the asylum workers. There's no question of his innocence. He did it, the blood is all there. The only thing to do with Dean Winchester gone crazy is to put him down.
