This was so devastating. Poor Dean. I imagine that Dean really does think of the horrors he thinks Sam is going through every day...
I am a little confused by the end. I know he is in a asylum and about to get a shot of something, but you said your note that Dean knows what is going on. I thought Dean finally thought he was in Hell (or something like that) and now he was firmly grounded in his delusions, therefore he would probably act pretty normal/like he used to, except for what he saw(if he saw tortures etc) around him...?
